Default form

Every gated file works out of the box with Smart LeadLock’s default form, available on every plan including Lite. It’s a simple, sensible name-and-email form that requires no configuration.

What it includes

The default form collects a name and email address, validates the email format client-side, and stores each submission as a lead and a submission record, both visible under Smart LeadLock → Leads and Submissions.

When to use a custom form instead

The default form is a good starting point for most gated downloads. If you need extra fields, conditional logic, or a multi-step flow, build a custom form instead and reference it with the form shortcode attribute. See Custom form builder.
